Identification
We cannot do your return without both of these:
1. Picture ID’s for yourself and (if filing a joint return) your spouse
2. Social Security cards or ITIN cards for yourself, your spouse (if filing a joint return), and all dependents
If you don’t have your Social Security card, go to the local Social Security office (In Cedar Rapids, 3165 Williams Blvd SW) and apply for a replacement. You’ll be given a letter that you can bring in place of your card.
Income
1. All W-2′s for 2011 wages, salaries, and gambling winnings
2. Any Form 1099′s for 2011 income, including retirement and Social Security benefits, interest and dividends, and work done as an independent contractor
3. Information about any other 2011 income, including scholarships and fellowships, jury duty pay, alimony, and prizes and awards
Expenses
Any Form 1098′s or other information about expenses that may be deductible, including:
Home mortgage interest
Charitable contributions
Medical expenses and health insurance premiums
These are deductible only if they’re large–over 7.5% of adjusted gross income.
Long-term disability insurance premiums
College tuition
Child care and dependent care expenses
You will need the provider’s name, address, and Social Security number or federal employer ID number.
Student loan interest
Property taxes
Employee expenses, such as unreimbursed mileage and tools
Educator expenses
Alimony
Estimated taxes and back taxes paid in 2011
Gambling losses
School fees and pre-school and private school tuition*
Vehicle registration fees*
